A coven of pagan witches were allowed to use one of the underground vaults as a place of worship. They picked one of the nicer, drier ones and built themselves a temple in there. They built a stone circle and placed in it several religious artefacts. They also placed a large upright mirror in one corner. The coven leader, George Cameron, had always been able to detect friendly presences in the vaults, but now he was picking up on something not quite so nice, and he decided to spend the night down there, in order to challenge whatever it was he could feel (not the easiest thing to do, considering how dark and dank the vaults are).
He positioned himself within the protected stone circle for the night. Good thing too, because it wasn't long before he heard something crawling about near the mirror in the corner. But whatever it was, it wouldn't cross his circle, and George came away unharmed. From then on though, people would complain that the mirror made them feel uneasy. A few claimed to have seen a white shape in it. A psychic visited, and voiced the obvious; the mirror was acting as a door.
George didn't want to give up his temple just yet though, and cast protection spells upon the mirror. It didn't work. The wall nearest the mirror became wet and began to smell. George agreed to shift the temple to another vault - taking the mirror with him. The original vault dried up, but the new one started to go the way of the previous. On various occasions the mirror was found face down on the ground, even though nobody could've possibly gotten in, and finally it was discovered on the ground broken. The man who was given the task of disposing of the mirror was hit by a car a few days later.
